Perimenopause and Menopause: When to Consider a Hormone Evaluation

Davonna “Dee” Wilson, DNP, PMHNP-BC, APRN, FNP-C

Perimenopause and menopause are natural stages of life, but the experience can be very different from one person to another. Some people notice only mild changes, while others experience symptoms that significantly affect sleep, mood, energy, sexual health, or everyday well-being.

Understanding what is happening and discussing your symptoms with a qualified healthcare provider can help determine whether further evaluation may be appropriate.

What Is Perimenopause?

Perimenopause is the transition leading up to menopause. During this time, hormone levels can fluctuate and menstrual cycles may become less predictable.

The transition can begin years before menopause, and symptoms may change over time.

Common Symptoms During the Menopause Transition

Experiences vary, but symptoms may include:

  • Changes in menstrual cycles

  • Hot flashes or night sweats

  • Difficulty sleeping

  • Changes in mood or irritability

  • Vaginal dryness

  • Changes in sexual health

  • Difficulty concentrating

  • Changes in energy

  • Changes in body composition or weight

These symptoms can have many possible causes, so experiencing them does not automatically mean they are caused by hormonal changes.

Why an Individualized Evaluation Matters

A hormone evaluation should consider more than a single symptom or laboratory result.

Depending on the individual, an evaluation may include a discussion of symptoms, menstrual and reproductive history, medical history, current medications, lifestyle factors, and relevant laboratory testing when clinically appropriate.

This helps the provider consider hormonal changes alongside other possible contributors to symptoms.

What About Hormone Therapy?

Hormone therapy may be considered for appropriate patients based on symptoms, medical history, individual risk factors, and clinical evaluation.

Treatment is not appropriate for everyone, and decisions about hormone therapy should be individualized between the patient and their healthcare provider.
